US Family Health Plan is an additional TRICARE Prime option available through networks of community-based, not-for-profit health care systems in six areas of the United States. You must be enrolled in the Defense Enrollment Eligibility Reporting System and reside in the one of the designated US Family Health Plan service areas.

The US Family Health Plan is the only TRICARE Prime program that offers benefits to beneficiaries age 65 and over, regardless of whether or not you participate in Medicare Part B. Similar to TRICARE Prime, the US Family Health Plan offers an option health care with few out-of-pocket costs.

"Where do I get medical treatment under US Family Health Plan?"

With US Family Health Plan, you do not access Medicare providers, military treatment facilities (MTFs) or TRICARE network providers. Instead, you and your family receive your care from a primary care physician that you select from a network of private physicians affiliated with one of the not-for-profit health care systems offering the plan. Your primary care physician assists you in getting appointments with specialists in the area and coordinates your care.

"Does US Family Health Plan cover my prescriptions?"

US Family Health Plan allows you to benefit from the same level of prescription coverage as the TRICARE program, and at the same costs. However, you may only use your US Family Health Plan's list of retail or on-site pharmacies and the US Family Health Plan mail order service instead of TRICARE retail, mail-order or MTF pharmacies.

"Does US Family Health Plan cover dental and eyecare?"

Each US Family Health Plan site offers enhanced benefits and services such as discounts for eyeglasses, hearing aids and dental care. These enhancements will vary by US Family Health Plan site.

"What if I change my mind about the US Family Health Plan?"

If you move or disenroll from the US Family Health Plan, you may choose any TRICARE program that you are eligible for and that is available in your area.

"How easy is it to participate in US Family Health Plan?"

There are no claims to file when plan-approved providers are used. Enrollment is required. You may enroll at anytime throughout the year and enrollment is automatically renewed each year unless you take action to disenroll. Enrollment fees, if applicable, may transfer to another US Family Health Plan location or to TRICARE Prime.

"Who is eligible for US Family Health Plan enrollment?"

  • Active duty family members

  • Retirees and their eligible family members

  • Survivors

  • Former spouses

  • Medal of Honor recipients and their families

  • Family members of activated National Guard or Reserve Members
You are encouraged to enroll in the US Family Health Plan as a complete family unit, but you may enroll on an individual basis. When you enroll in the US Family Health Plan, you are committing to the plan for one year unless moving out of the area or disenrolling for another qualified reason.

"Should I also get Medicare Part B?"

You do not need to have Medicare Part B to enroll in the US Family Health Plan. However, if you do not enroll in Medicare Part B when first eligible, and subsequently choose to do so, you will pay (in addition to the normal Medicare Part B monthly premium) an annual 10% penalty for each year you were eligible to enroll in Medicare Part B and did not.

In addition, you will only be able to in Medicare Part B enroll during the general enrollment period, January 1-March 31, of each year, and your Part B benefits will not be effective until July 1 of that year. You are encouraged to enroll in Medicare Part B when first eligible.
